UA NSSLHA Application

Submitting my application certifies that I agree with the following statements: 

1) "I understand that my NSSLHA/MAPS membership application will not be completed or processed until I have paid my yearly membership dues of $10.00 to the University of Arizona NSSLHA.  (The only individuals waived of this application fee are NSSLHA officers, NSSLHA committee chairs, and MAPS mentors due to their extensive service to NSSLHA)."

2) "I understand that the security of the information I provide here is managed by Zoho Creator and will refer to their use, terms, conditions, and privacy statement for security information."
